• Aged 65 and over by 31 March 2024
• Aged 6 months to 64 years at medically increased risk
• Living in a care home for older adults
• Frontline health or social care worker
• Age 16-64 and a carer
• Aged 12 to 64 and live with someone with weaked immune system

Please bring along your NHS number – if you have one.
